SPLC Awarded OTF Capital Grant

The Ontario Trillium Foundation (OTF) has generously awarded SPLC with a grant to improve our Adult Day Program space. Building inclusive and engaged communities together, this initiative will help people who are isolated have connections in their community, and will have an impact on the lives of 3,400 people in the community. We are extremely grateful to OTF for their support.

The work of SPLC GAIN Community Team Pharmacist, Jenny Wong has been published as a Clinical Feature in Pharmacy Practice + Business (January / February 2019 vol.6 No.1) publication.The feature aims to help pharmacists enhance the level of care for seniors, and is titled "Optimizing Care of Older Patients: Tips, tools and resources to help pharmacists with geriatric care.

Shirin Vellani

Shirin Vellani, SPLC GAIN Community Team NP is a Published Academic Author

Shirin Vellani is one of the longest standing members of SPLC's GAIN team, and a talented Nurse Practitioner. Recently, she added published academic author to her list of accomplishments. Her journal article titled "Identifying and understanding the health and social care needs of older adults with multiple chronic conditions and their caregivers: a scoping review," seeks to understand what the health and social care needs of seniors with multiple chronic conditions are, as well as how societal factors like education and socioeconomic status can influence these needs.
