Local Health Integration Network (LHIN)
Local Health Integration Networks (LHINs) are organizations established by Government of Ontario that help connect people with the right healthcare services for their particular needs. They connect patients and caregivers with an extremely wide range of health related services, such as assistance with diabetes management, care for dementia patients, referrals to long-term care homes, and much more. If you or the older adult under your care requires health related support services, and you are unsure of where to look, the LHIN is an excellent place to start.
