What support is available for you?
Integrated Care Managers help older adults and their caregivers navigate the health care system and provide ongoing support when facing life changes. The primary role of Care Managers is to keep you connected with the care and services you need. Care Managers empower you to be in control of your health, by enhancing decision making skills and providing support to develop effective coping strategies.
Care Managers are taking new referrals to provide older adults, aged 55+, with one-on-one supportive counselling, information, and referrals to other resources so they can live safely at home.
We also provide services to help navigate next steps as individuals/families work through their challenges brought on by changes in physical health, mental health, social and/or financial circumstances.
The SPLC team communicates in English, Chinese (Cantonese and Mandarin), Korean, and Arabic.
What does this support look like?
We ensure you receive the following:
- A personal assessment to identify needs, concerns, and risks
- A Care Plan which is developed in partnership with you (and your caregiver, if needed)
- Referrals to community resources or programs; in-person counseling or telephone support counseling
- Client advocacy & Crisis intervention
COVID-19 protocols
Where possible, support is provided via phone and healthcare-compliant Zoom platform.
For in-home or in-person services, our staff are expected to:
- Conduct self-screening prior to working
- Screen clients for COVID prior to in-home/in-person service delivery
- When possible, staff will reschedule clients if they do not pass the COVID screening. If it is not possible to reschedule, staff will wear full personal preparedness equipment while providing services
- In our office, tables and chairs are sanitized before and after client
- Wear masks at all times during service delivery and when in the office
during service delivery, and where possible, service is delivered either between plexiglass or staff to keep 6 feet of physical distancing
Virtual programs
Our virtual/phone group programs continue including:
- Cancer Support Group in Cantonese and Mandarin for cancer survivors, cancer patients and their caregivers to provide mutual support and a platform for speakers, such as dietitians, to talk about ways to stay healthy
- Wellness Group in English and Cantonese includes a brief presentation to discuss ways to share experiences and learn new ways to maintain good mental health during times of physical distancing amongst seniors
- Spiritual Support Group in Cantonese is a group in partnership with St. Paul’s L’Amoreaux Church. This group conducts bible studies, sings hymns together, etc.
- Virtual Counselling can be helpful for a wide variety of issues, such as COVID-19 anxiety, family conflict, grief, stress, life transitions, depression, and much more.
