We are delighted to announce St. Paul's L'Amoreaux Centre received funding from the Central East LHIN to run a GAIN Community Clinic!
Part of Regional Specialized Geriatic Services, a Geriatric Assessment and Intervention Network (GAIN) Clinic provides a comprehensive assessment of seniors with more complex needs. It also provides access to an inter-professional team who help develop personalized care plans (with input from both the senior and their caregivers) that provide recommendations about what services and supports may help seniors continue to live at home and maintain or improve their health.
Seniors who attend a GAIN Clinic can live at home or in a retirement residence, are typically 75+ and would benefit from having an assessment by an inter-professional geriatric team because of any of the following needs:
- Multiple complex medical, functional, mental health and psychosocial problems
- Recent functional or cognitive decline
- Frequent falls, or those at risk of falling
Our GAIN Team will include:
- Nurse Practitioners
- Community Care Access Centre Case Managers
- Physiotherapists
- Occupational Therapists
- Social Workers
- Pharmacists
- Personal Support Workers
- Geriatrician and physician support
- Administrative team members
